Please note: This form is for COGS Members and Associate Members only. 


Completing your Listing Information for the web:
  1. Complete the form and add a 2nd page with a short description. This is your introduction to people about your business. Make it personal, engaging, and interesting, this is your opportunity to talk to someone about what you do and why you do it.
Adding Photos: (opTIONAL)
  1. Provide up to (2) photos that relate to your business, or you may also provide a photo of yourself, a headshot is best.  See below for graphic sizes and instructions for providing quality photos. 
  • If you are scanning and sending us a photo by email, please scan it at 300 dpi and size it according to the specifications shown below. Email your photos to,
  • If you have an original photo you want to use you can send it to us. We will scan it, size it correctly, and upload it to the Directory. The cost for this service is $15 per photo. Checks should be payable to Maryle Malloy and included with the Listing form and your photos. Mail to: Church of the Good Shepherd, 7834 Tennyson NE, Albuquerque, NM US 87122.  Your photo(s) will be returned within 30-days of receipt. 
  • If you want to use a photo from your smartphone or camera you can email it to   She will resize it and optimize it for the web. The cost for this service is $10 per image. You will receive an invoice from her once the image(s) are loaded to the Directory. 

For horizontal layout the recommended size is 1,024px wide by 576px tall.  For a more vertical/square layout of images, the recommended size is 600px wide by 600px tall.
